I get the first two, but don't know what is the "hack" used for?

You would have to ask the designer of that specific key set about the "Hack" key.

I would suppose that it is mostly for novelty, or for mapping to whatever. Similarly, the "Code" key is not a standard key either.
"Code" was first found on typewriters that had Cherry MX compatible keycaps, and people thought it would be cool to have one on replicas of typewriter keycaps (https://geekhack.org/index.php?topic=79311.0). Then it lived on.

The two are used in 68 keyboard

r4 hack is for people who want to use it for fn or a 1u windows key on the bottom row

just a generic legend
