Brand new in fully sealed box, but the box has almost two decades of multiple tears, scuffs, and discoloration where the gloss coat has worn off. The box also has some labels on it (see pictures). ALL seals (IBM and IBM's QC seals) are still intact.

ALL seals (IBM and IBM's QC seals) are still intact. The box has not been opened, thus the keyboard has never been used or tested.
Keyboard was built/"born" September 27, 1997 according to the box label (inside manufacture label is unknown but should match). Made in Mexico plant. It is likely Maxiswitch manufactured for IBM (as most Model M13's were).
