I don't know how this happened, but what can I do to fix this or avoid it in the future.

Looks like you put the switch top on back to front and the leaf is stuck in the LED slit?  You might be able to pull it out and put it back where it should be (there's a tittle hole for the leg to locate it), depends if you pushed hard enough to bend it.
