The address field in the form is a bit small.
What is "Option 1" checkbox?
I do not see what your seeing on the form....
Must have been a glitch in the matrix.
The address was a single short field like phone number.
Between comments group box and submit button was a checkbox labelled "Option 1".
They are both as they should be now.
