Most elegant way to set up Programmer Dvorak on a Kinesis

Hi, brought about by the anishtro layout thread here https://geekhack.org/index.php?topic=74546.0, I was wondering if someone has set up Programmer Dvorak on a Kinesis, and what combination of remapping and macros worked most efficiently. It appears remapping takes precedence over macros, making the order things are done important, so if someone has done this before I would be happy to see the directions. Here is Programmer Dvorak on a standard keyboard:

And something closer to Kinesis:

You may not fit to the maximum safe macro count of 24. Or if you do fit then just barely. You can increase macro count to 38 or 48 but then you risk that the firmware will not detect some keystrokes if you can type quickly enough. I did not try Programmers Dvorak.
