a) Laser cutter typically
2) Size of each hole is shown in the Simple Questions thread. I'm not sure about the spacing between each switch but that might be in the CAD Resources thread.

Laser cutting machines aren't cheap, the cheapest I'm seeing is around $3000. Most likely you'll have to go find a machine shop that has one and have them cut it for you. Lasers can cut the holes in the plate and then cut the plate to the proper dimension.

Yet again, your second question isn't making sense, to the best of my understanding, I already answered it. I don't understand what you mean by "how to holes box exactly?" Does that mean how to cut them? Or how the holes are patterned? Or what?

I'm still unsure what you mean by "them" but if you're just making a plate, you can either cut it with a laser cutter, or mill it with a mill controlled by CNC or by hand. You can shape the plate itself and cut out the holes with the same tools.
You can build your own 3 axis mill but it won't be cheap either; will cost a lot of time and money. I doubt you'll get a plate that will work with hand tools.

It will be VERY difficult and practically impossible to cut a plate with a hand tool. If you have access to a manual mill where you turn wheels, it won't be as hard but it will remain freakishly tedious to the point where the average human would die from boredom before having cut half the holes.
You pretty much need a CNC laser cutter. A CNC waterjet cutter will suffice and a CNC mill is also able to do it but not as easily. I recommend you to draw it digitally, have someone take a look at it and then order it at a shop which cuts things for you.

I don't see why it would be impossible to make a plate with hand tools. Tedious as hell, but totally possible. Get extremely accurate measurement tools (you're gonna want to measure to something like a tenth of a millimeter) and fine blades and spend enough time on each hole to make it perfect. Honestly it's possible that if you just worked a minimum wage job during the time you were hand making the plate you could afford a laser cutter, and even if it's WAY faster (I'm talking exponentially faster here) than I'm thinking it would still be enough to pay someone else to cut it.

It's not impossible to do it by hand; just a royal pain in the ass and super tedious.

Pololu (http://www.pololu.com/catalog/product/749) is what you're looking for. They offer very cheap laser cutting and nice variety of materials and colors.

Laser cutting's not the only solution. Waterjet cutting is pretty good, too.

what about a Dye grinder get a sheet and start shaving away, take forever but technically possible.

I would also recommend using AcidFire's custom acrylic service.
As for design related stuff the CAD resources thread has the template files, the spacing between the keys depends on the size of the two keys.
