Title: Relegendable insert template?

Hi all, I'm looking for a template for the little slips of paper you put in relegendables (DCS to be specific)
If one of those doesn't exist then if anyone has a general idea of how small I'd have to make an image to put it into the caps that'd be great, I really suck at eyeballing things lol.
