Every single ergonomics guide recommends typing with the shoulders relaxed. Is that even possible!?

Sure, that's how I do it. Why is it problematic for you?

Well, although I consciously start out with my shoulders relaxed, after five minutes I find that they are tense and hunched. Besides don't movements of the forearms and fingers systematically tense the shoulders?

sounds to me that your source material is tensing you up, like your typing with rage type of deal.
I'm not saying your nerd raging when your typing but you should just try to examine what is happening when you are typing i guess. at least from my point of view i only get tense when i get client emails and usually those emails are the enfuriating kind.

How high is your chair compared to the table?

I think your chair back needs to be wide enough to stop your elbows from being pushed backwards by the turning moment of force from the weight of your forearms.

(I must learn how to shorten my sentences.)

The height of my chair is OK, I guess, as my forearms are level with my wrists when my shoulders are relaxed. The difficult part is keeping them this way after I have started typing.

I think Lanx makes a good point. Most of what I type is code, and when I'm concentrating on the problem at hand, it's easy to forget about good posture.

adjustable elbowrests should eliminate this problem
