Open it up.
I'm not convinced that it was made by or for IBM, or made in the USA, at all, unless there's some Chinese town that was renamed Usa, so that China could get the same advantage as Usa, Japan.

That doesn't look like anything that I've seen from Unicomp before, though.
-
humm is your friend sickchief? O_o that looks like a box I had here and sent him^^.
-
I also initially thought a Unicomp, since that's similar to their labeling scheme, but...you're gonna have to open it to find out for sure.

the corners are square - real US IBM labels have rounded corners and the cut job looks to be by hand. There doesn not appear to be a glossy clear top lable (protects the paper label). it looks like there might be some sticky residue left from a previous lable that was taller and narrower than this lable - like the UK lables.
Unicomp did have a model 101 (same as the ibm but with different labels). i agree that this is probably a Unicomp referb. with a referb, you might have mix/match parts from various Model M, so i would not worry too much about it. the only problem is how to re-lebel it (if you should want to).

if this is the board i think it is its one of the berlin group buy boards. If not, im stumped.
