Title: How to fill out FCC-740 form for importing keyboard? Status: RESOLVED
Post by: Heliosphere on Tue, 16 December 2014, 14:34:35: I bought a FC660C from Korea and the seller just shipped it out but FedEx called me saying I need to fill out a FCC-740 form before it can be shipped. Has anyone filled out one of these before and can help me out understanding how to complete this?

The form looks like this:

Update: I filled out the form and it seems to have gone through. For future reference, I will list some of the info in case anyone else is planning to import a FC660C...

Device Model: FC660C
Trade Name: Leopold
FCC ID: RPKFC660C
Manufacturer's Name and Address (This one I'm not 100% sure about, use at your own discretion):

For part II, mark number 1: "The FCC has issued a grant (...)"

And you don't necessarily have to fill out the first four entries regarding entry or tariff number.

It's good to know the first four sections are optional, but I'll contact FedEx to see if it's acceptable. I've managed to find the rest of the information but I'm having trouble with Leopold's address, which doesn't translate very well through Google. If anyone knows a direct phonetic translation of their address, please post it!
