So.. Like I never bought one of these..

Are they supposed to stay plugged into the phone?

Is it constantly trickle charging the phone's internal battery ? or is there a switch..

On/off switch. When you charge it with the case on, it will fill the phone's battery first, then the case.

I believe most do. Even those I purchased 3 years ago have switch, so all the newer ones gotta have it too.

My otterbox battery case has an on/off switch and it's pretty new (few months old).
