Dear right handed geekhackers, how can you possibly scribble, underline and take notes when your mouse is on your right, and when your 100% keyboard extends much more on the right side than on the left?
The "standard" setup was clearly designed by some left handed engineers. ;D

So true. This is the reason why I use a 60% and put my numpad on the left side :D

60% is the answer :)
Was thinking about this the other day though when an IC about a new 60% board was discussed. Like, most of the ergonomic issues of full-sized boards would disappear for left-handed users.

I prefer a keyboard tray, they you have your whole desk clear for scribbling.

As a left handed, I still prefer to write on the left of the keyboard, if I was right handed, the tray could be a solution. Anyway they are often too wobbly, not stable enough...

The cheap ones are wobbly, but the good ones, like the rail mounted ones from 3M, are very stable.

Why would I want the keyboard and the other stuff in the same height in the first place? I touch type and that's better with the keyboard slightly below elbow height (in order to keep wrists straight while typing). That's not the case with handwriting. Of course I have the keyboard on a tray or in my lap, thus lower, while printed material is in my field of vision, thus closer to screens. (Ignoring the fact, that I prefer to use a pen-enabled tablet (i.e., a graphical tablet or a Tablet PC) for handwriting.)

Bought a numpad for work just for this. Mouse+numpad control will get me where I want in excel spreadsheets most of the time.
