Full-size buckling-spring Unicomp Model M made for GE/Marquette cardiology systems. Lots of fun (dye-subbed) OEM sublegends, not to mention the wild yellow tenkey.
This one also has a Unicomp-branded overlay, which is somewhat unusual; being OEM boards, most have generic overlays like this one, which I posted about here (https://geekhack.org/index.php?topic=77522.0) in Nov. '15:
