The label's faded, for some reason (Unicomp just not watching their ink levels, I imagine—LOL). But it'd be easy enough to DL a hi-res image of one (e.g. this one), mod it appropriately, and print your own.

I have one like this, a year or 2 older. People who complain about current Unicomp QC should pounce on this.
