This is a Chicony(?) KB-7001 branded as an Addison. If it's like the other 7001s, it should come with complicated white ALPS switches. This guy is also available directly as a Chicony or as REVEAL / PC Accessories rebrands.
Starting at $19.99 + $10.00 shipping. Keys are probably shiny from wear.
EDIT: Actually, I probably shouldn't call this a split ergo to avoid confusion with actual two-part ergo boards.

Missing 4 LEDs apparently. Should be USB. Keep in mind that plate-mounted MX switches will need to be desoldered if you want to open them up.
