MediaMonkey is a decent alternative to iTunes.

As for Foobar, I've never used it with my iPod, but it does have a menu for iPod, I guess it works the same with iPhone. Should be File > iPod/iPhone > Synchronize, Manage Content or whatever you want to do.

Edit: I just found out you need a plugin to use it with an iPod, not sure if it works with an iPhone, but take a look around the hydrogenaudio forums.

I'd recommend SongBird. It has a massive range of plugins that will allow you to sync to iDevices.
I'm typing from a CM Quickfire with MX Blues.I'm also on a constant search for cool keycaps.
PM me if you've got any you'd like to send to Australia, I'm happy to buy!

What are the problems you're having with iTunes?
It may not be worth looking for an alternate right now when iTunes 11 is due later this month.

Check this thread and see if that's any help.
From what I gathered you need foo_dop and iTunes installed for it to work properly with iOS 5, if you're running 4 you should be able to use foo_dop without iTunes installed it seems.
