I got some stuff from Shapeways just to see how they were. The Topre profile FNs fit pretty well at least. Unfortunately they are a bit flat and don't have that curvature/scoop to that you would typically see. The skulls are a bit of wonky fit though, might try and tinker with them.

These are Shapeways caps, just painted. I thought these were pretty neat, found them on ebay.

Yep, they do hurt your jaws like hell. I had to wear braces a little over 5 years (3rd - 8th grade). When I finally got them taken off it was one of the happiest days of my life.
