Diagnosed ENTIRE audio chain.

Computer, sound card, cpu, mobo, the piano, cables, subs, disassembled speaker crossover to verify component tolerances.

Turns out it was just a loose cone... took all day....

Went in exactly the wrong order.

How does a cone become loose?

Probably over time the wood expands and contracts from humidity, years of it can loosen the screw holding the cone. Cones actually have quite alot of mass.

Ah, you mean the woofer's frame was loose.  Depending on your climate, if you have AC or not, I can see how screws may loosen over time.  Fortunately, I haven't had this issue.

Also, I would see the vibrations over time would loosen the screws.  I would have checked the woofer/speaker first.
