How to fill out FCC-740 form for importing keyboard? Status: RESOLVED

I bought a FC660C from Korea and the seller just shipped it out but FedEx called me saying I need to fill out a FCC-740 form before it can be shipped. Has anyone filled out one of these before and can help me out understanding how to complete this?

Update: I filled out the form and it seems to have gone through. For future reference, I will list some of the info in case anyone else is planning to import a FC660C...

And you don't necessarily have to fill out the first four entries regarding entry or tariff number.

Apparently computer peripherals may create radio interference because they send electrical signals, at least according to the FCC.

It's good to know the first four sections are optional, but I'll contact FedEx to see if it's acceptable. I've managed to find the rest of the information but I'm having trouble with Leopold's address, which doesn't translate very well through Google. If anyone knows a direct phonetic translation of their address, please post it!

I completed the form and sent it in. The FedEx agent hasn't said there's any problem so I assume it's gone through. I've updated the OP with some of the info for future reference.
